ABSTRAK
Pencatatan berkas rekam medis di RSUD dr. Abdoer Rahem masih banyak yang tidak lengkap, tidak terbaca dan terdapat lembar formulir yang kosong. Analisis kuantitatif pada berkas rekam medis sangat diperlukan untuk mengidentifikasi ketidaklengkapan berkas rekam medis sehingga dapat menghasilkan daftar kekurangan yang bisa dilengkapi oleh penyedia asuhan kesehatan. Berkas rekam medis yang tidak lengkap tidak dapat dijadikan sebagai alat bukti pelayanan dan tidak mengandung nilai ALFRED didalamnya. Tujuan dari penelitian ini adalah menganalisis kuantitatif berkas rekam medis kasus diare infeksi pada anak di ruang teratai RSUD dr. Abdoer Rahem Situbondo Triwulan I tahun 2016. Jenis penelitian ini deskriptif kuantitatif dengan pendekatan retrospektif. Perolehan data dari penelitian ini melalui observasi dengan lembar checkhlist. Hasil yang didapatkan dari observasi berkas rekam medis di RSUD dr. Abder Rahem Situbondo angka ketidaklengkapan tertinggi pada review identifikasi terdapat pada lembar pemberian obat injeksi/rectal/inhalasi yaitu sebanyak 9 (10,6%) berkas rekam medis. Review laporan-laporan yang perlu terjadi pada resume klinis sebanyak 24 (28,2%) berkas rekam medis. Review otentikasi terdapat pada lembar pemberian obat injeksi/rectal/inhalasi dan perencanaan pasien pulang mencapai 30 (35,3%) berkas rekam medis. Review cara pencatatan terdapat pada resume medis mencapai 36 (42,4%) berkas rekam medis. Perlu memberikan pemahaman mendalam dari catatan medis oleh dokter dan kesehatan pekerja untuk lebih bertanggung jawab dalam mengisi catatan medis.

ABSTRACT
Recording of medical record file in dr. Abdoer Rahem many incomplete, unread file and there is an empty form sheets. The quantitative analysis on medical record file is needed to identify the incompleteness of medical record file so that it can make a list of deficiencies that can be fitted by a health care provider. Iincomplete medical record file can’t be used as evidence of service and does not contain the ALFRED value. The purpose of this study was to analyze quantitative medical record file of infectious children diarrhea cases in the lotus dr. Abdoer Rahem Situbondo first quarterly of 2016. This study was descriptive quantitative retrospective approach. Acquisition of data from this study through observation with checkhlist sheets. Obtained from medical record file observations in dr.
Abder Rahem incompleteness Situbondo is highest numbers on the review
