Evaluation of Medical Record Management at Puskesmas Klatak Banyuwangi with Accreditation Guidance Year 2015 Yulisa Rahmawati Samsul Medical Record Study Program Department of Health ABSTRACT The management of medical record at puskesmas is used to support the achievement of administrative in order to reach the goal to improvehealth service quality at puskesmas. Means in improving the quality of Puskesmas services must be implementing accreditation. Puskesmas must accredit in every 3 (three) years. Medical record information management is one of the criteria in accreditation assessment. This research aims to evaluate the management of medical record at Puskesmas Klatak Banyuwangi with Puskesmas accreditation guidance’s years 2015 to know the condition of medical record in following accreditation assessment. The method of this research is qualitative research using guidelines for puskesmas’s accreditation. The methods of data collection in this research using observation interviews. The result of measurement of achievement of medical record shows that medical record management has completed, in accordance with the requirements of competence. Achievements obtained by Puskesmas Klatak (69.23%) is in the Partially Achieved criteria for the Manajemen Pelaksanaan Klinis (MPLK) sub-section of the Medical Records Information Management standard so that it can be classified as Basic management, Primary or Primary graduation rates. The conclusion of this research is preparation of accreditation based on medical record information management element at Puskesmas Klatak is need to be updated, socialize and apply standard operating procedure (SOP) or other regulation exist so that can cover all aspect in accreditation standard.
