Desain Formulir Pasien Gawat Darurat Di Rumah Sakit Umum Daerah Besuki Tahun 2016 (Githa Ayuning Lestari) (Design of Emergency Patient Form at Besuki General Hospital in 2016) Githa Ayuning Lestari Medical Record Study Program Department of Health ABSTRACT Emergency forms at the Besuki Regional Public Hospital still use an outpatient form in identifying emergency patients so they do not conform to existing standards and do not comply with user requirements. The purpose of this study is to design a special form in patients with IGD at Besuki Regional Public Hospital. The type of this research is descriptive qualitative with observation method, interview and Focus Group Disscussions (FGD). The emergency patient form design result is based on the physical aspect of paper material using HVS 80 gram, A4 paper size with white color and black ink. From the anatomical aspect on the head of the form there is the addition of the logo of the district government, the introduction of the title of the form, there is a change of instruction that is by checking the box available, the body of the form is tailored to the needs of the user and the cover is added with the name of the doctor while to fill the form tailored to the needs of the user in the form of triage, ABCDE concept on the triage to the final condition of the patient when leaving the IGD room adjusted to PERMENKES No. 269 / PER / III / 2008 and added cover in the name and signature of the treating doctor at the ER. Suggestions from the emergency form design results are expected to be used into consideration in the planning of emergency provision procurement at Besuki Regional Public Hospital.
