PERANCANGAN MAP DAN PEMBATAS FORMULIR REKAM MEDIS DI RUMAH SAKIT TK.III BALADHIKA HUSADA JEMBER Pembimbing (Atma Deharja, S.KM, M.Kes) Monica Ita Triwardani Program Studi Rekam Medik Jurusan Kesehatan ABSTRAK Kejadian missfile di Rumah Sakit Tk.III Baladhika Husada Jember terjadi diantaranya karena belum adanya kode warna pada map rekam medis. Kejadian missfile terjadi sebesar 0,8 %. Rancangan map rekam medis yang digunakan belum sesuai karena belum dilengkapi tab, sehingga petugas membutuhkan waktu 12-15 menit untuk mengambilan berkas rekam medis. Map yang digunakan sering mengalami kerusakan sebesar 62 %, sehingga formulir rekam medis didalamnya rusak. Kesalahan urutan letak formulir sering terjadi sebesar 40 % yang mengakibatkan formulir rekam medis didalamnya tercampur, sehingga dibutuhkan pembatas formulir rekam medis. Penelitian ini bertujuan untuk merancang map dan pembatas formulir rekam medis. Jenis penelitian dalam penelitian ini adalah deskriptif kualitatif. Unit analisis pada penelitian ini yaitu kepala rekam medis, petugas filling, petugas assembling. Metode pengumpulan data dalam penelitian ini menggunakan observasi, wawancara, dan brainstorming. Berdasarkan hasil penelitian, map dan pembatas formulir rekam medis berdasarkan aspek fisik kertas yang digunakan pada map yaitu art paper 150 gram dengan ukuran 33x23 cm berwarna hijau kuning. Pada pembatas formulir adalah kertas art paper 100 gram dengan ukuran 29,7x21 cm berwarna putih dan ukuran tab formulir 5x1 cm. Sedangkan untuk aspek anatomi dan aspek isi pada map dan pembatas formulir rekam medis menyesuaikan dengan kebutuhan yang ada namun tetap sesuai dengan tujuan map dan pembatas formulir dibuat. Saran dari hasil rancangan map dan pembatas formulir ini dapat digunakan sebagai bahan pertimbangan dalam perencanaan pengadaan map dan pembatas formulir rekam medis baru sehingga mempermudah pekerjaan petugas assembling dan filling. File placement sequence errors often occur by 40% resulting in the medical record form in it mixed, so it is necessary to restrict the medical records form. Based on research results to design a map and dividers of medical records on the physical aspect of paper used on the map of art paper 150 gram with size 33x23 cm yellow green. The dividers is a 100 gram paper art paper with 29.7x21 cm white color and 5x1 cm form tab size. As for the aspect of the anatomy and aspects of the contents on the map and the dividers of the medical record adjust to the needs that exist but still in accordance with the purpose of the map and the dividers is made. Suggestions from the map and the dividers are expected to be used as a consideration in the procurement of the map and the dividers of the new medical record to facilitate the work of assembling and filling officers.
