Analysis Of Registration Officer Behavior That Causes Duplication With Abc Theory (Antecedent, Behavior, Concequences) In Pakem Gladak Health Center. Atma Deharja S.KM, M.Kes (chief of counselor ) Diananda Wegi Putri Bareta Study Program Medical Record Majoring of Health ABSTRACT Public Health Center of Gladak Pakem is one of the Public Health Center in Jember that inpatient and outpatient services in Pakem. Based on a preliminary survey conducted by researchers in July 2017 at Public Health Center of Gladak Pakem 6 officers, there were duplicated medical record files, 1 patient had 2 medical records just like an example in patient X with the number of medical record numbers 2189 and 2760 from 50 medical record files duplication there are 20 files that are duplicated in the outpatient and inpatient registration section. The purpose of this study is to analyze the causes of the registration numbering system at Publick Health Center of Gladak Pakem. The subject of this research is 1 outpatient registration officers and 5 inpatient registration officers. The type of research used in this study is qualitative research. The instrument used consist of questioner, interview guidelines, observation and brainstorming. The results of this study are of ficers never participated in the training about medical records, there’s not SOP about numbering system and then there’s not a punishment. Beside on the result Public Health Center of Gladak Pakem advice to give a training that is done 2 time 1 year, compile the SOP about numbering system, and recommend give a punishment in the form of reprimand for officers who made mistakes on duty.
