ANALISIS PENYEBAB TIDAK TERLAKSANYA RETENSI DAN PEMUSNAHAN BERKAS REKAM MEDIS DI RUMAH SAKIT DAERAH KALISAT JEMBER Atma Deharja1, Intan Nova Cristin Hariyanti2 Program Studi Rekam Medik Jurusan Kesehatan Politeknik Negeri Jember atma_deharja@polije.ac.id Abstrak Retensi dan pemusnahan berkas rekam medis umumnya dilakukan setiap 5 tahun. Hasil survei pendahuluan pada tanggal 14 Maret 2017 di Unit Rekam Medis Rumah Sakit Daerah (RSD) Kalisat Jember yang sedang menjalankan proses retensi berkas tahun 2007 hingga tahun 2011, retensi akan dilakukan di tahun 2016. Tujuan penelitian ini adalah untuk menganalisis pelaksanaan retensi dan pemusnahan berkas di rumah sakit daerah kalisat jember. Berkas rekam medis mulai tahun 2012 hingga 2016 masih dalam penyimpanan. Jenis penelitian ini adalah penelitian kualitatif dengan teknik pengumpulan data wawancara, observasi, dan Focus Group Disscussion. Hasil dari penelitian ini adalah didapatkan 3 prioritas penyebab masalah teratas yaitu dari segi sumber daya manusia, pendanaan, dan mesin. Yang paling menyebabkan tidak terlaksananya retensi dan pemusnahan berkas ialah dari segi sumber daya manusia sehingga ditentukan upaya perbaikan untuk menambah sumber daya manusia di unit kerja rekam medis. The purpose of this study is to analyze the implementation retention and destruction of files at the Jember District Hospital of Jember. Medical recordfilesfrom 2012 to 2016 are still in storage. The type of this research is qualitative research with data collecting technique of interview, observation, and Focus Group Disscussion. The result of this research is got 3 priority cause of the top problem that is from human resource, funding, and machine. The most cause of the failure of the retention and destruction of the file is in terms of human resources so that determined efforts to improve the improvement of human resources in the medical record work unit.
